Game unplayable after last patch on pc

Before the last patch I was playing at High-Medium quality with 60 fps without drops.

Now I can't reach 60 in town. I lowered the quality to low and I'm between 10-30 fps with continuous changes without stability. Anyone with the same issue? It feels the game is getting worse not better!
