Ccw paper work help

I keep seeing people saying you need 4 references but the paper work I printed out from the government website only has spots for three . Also I keep seeing I need to fill out 3 separate but identical forms but also heard I only neeed does anyone have any clarity about this . A lot of the documents online that are showing up are old .
