SLPA requirements confusion

Reached out to my state SLPA department because I am confused on what they actually require. I’m in KY, post-bacc degree in communication sciences and disorders but currently in grad school for SLP. They sent this but I truly do not understand exactly what they want. Especially the hours requirements. Can I just apply for the interim license or do I need something further?
