Question about Harbor Pilots

After watching "Mighty Ships" on the Smithsonian channel, i get the impression that harbor pilots are just another way for a port to make money from a ship ( or are they included with dock fees?). With the technology available today, are the harbor pilots really needed on up to date ships? The captain is still ultimately responsible for the ship anyway. What am i ( as a land lubber ) missing? Thanks!
