Please help me identify things on my grandfathers plaque.

My grandfather fought in Korea and was I'm the battle of the Chosen Reservoir. That's about all I know about my grandfathers time in Korea. Please help me identify the things on this plaque my dad found. My grandfather never wanted to talk about his service so any info would be greatly appreciated
