Dry penis head (uncircumcised)

Since 2020 my the head of my penis had developed a dry and cracked problem (sometime itchy), which makes it difficult to roll up my foreskin

I have seen the doctor a couple of times, tried moisturisers, creams, tried wearing loose clothing and not jacked off, but nothing seems to work - why is this? Is there any advice and anything I can do at this stage? Will this last the rest of my life?
